PPO Network Dentists|
PPO network dentists agree to accept payment for covered services according to a discounted fee schedule, which means you’ll have the lowest out-of-pocket costs when you choose a PPO dentist. You’re responsible only for your deductible, coinsurance and any fees for noncovered services. PPO dentists will also file your claims with and receive payment directly from the Blues. Where permitted by law, PPO dentists offer the discounted fee schedule for noncovered services.
Blue Par Select Dentists
Blue Dental (Personal or through your employer)
Members who have Blue Dental plans personally or through their employers can choose a PPO dentist from the entire DNoA Preferred Network*. This Network includes dentists from a number of regional PPO networks, including DenteMax.*
BCBSM Medicare Advantage PPO
Members who have BCBSM Medicare Advantage PPO plans can choose a DenteMax PPO dentist who is designated to treat Medicare Advantage PPO members or a dentist who contracts directly with BCBSM in the Medicare PPO Network.
BCN Advantage (Medicare HMO)
Members who have BCN Advantage (Medicare HMO) plans must choose a DenteMax PPO dentist designated to treat BCN Advantage members. (Effective 1/1/13,you may also choose a dentist who contracts directly with BCBSM in the Medicare PPO Network)
Federal Employee Program
Members who have Federal Employee Program (FEP) dental plans must choose a DenteMax PPO dentist.
Affinity Discount Program
The Affinity Discount Program is a discount dental program that provides great value at no charge to Blue Cross Blue Shield of Michigan retirees. Members in this program receive discounts on dental treatment performed by participating DenteMax PPO dentists. Blue Cross Blue Shield retirees can choose any dentist, but they receive affinity program discounts only when they choose DenteMax PPO dentists who participate in this program. (Not all DenteMax dentists participate in the Affinity Discount Program.)
Most non-PPO dentists in Michigan participate in our Blue Par Select arrangement, which means they participate with the Blues on a per-claim basis. Blue Par Select dentists accept the Blues approved amount as full payment for covered services you’ll pay only your plan’s deductible, coinsurance, or any fees for noncovered services. Not all Blue Dental plans cover services provided by Blue Par Select dentists.
Check with your dental office to confirm participation before making an appointment or receiving treatment. Be sure to mention if you have a Medicare Advantage plan.
